Primary Care Doctors - Alamo, CA

"I had an appointment with Dr. Pham at rhe martinez VA hospital. 3 days before our first meeting she had already written a formal diagnosis for me for a mental illness which i dont have. I disnt have transportation to the hospital for the physical appointment so i had to cancel the appointment. But the note she wrote in my health record remained because hae wrote it 3 days before our appointment. She has never met me or spoken with me and she has never given me a psych evaluation. she wouldnt return my calls to explain why she made a ficticious diagnosis for me without basis. I had to call the office of inspector general to investigate that fraud and they had the VA remove her baseless diagnosis that she wrote 3 days before our first appointment which could not have been legitimate as she never met me or spoke with me and there was no evaluation. That is medical malpractice and fraud."
