Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Percutaneous Left Atrial Appendage Closure (LAAC)

Percutaneous left atrial appendage closure (LAAC) is a device implantation procedure that reduces the risk of stroke for patients who have atrial fibrillation, a condition in which the heart beats out of rhythm. Atrial fibrillation causes blood to collect in the left atrial appendage, a small sac located in the top left chamber of the heart. The blood that collects in this appendage can form clots and cause stroke when pumped out of the heart.

To implant the device, a long, flexible tube, called a catheter, is inserted into a large vein in the groin and advanced to the heart. Once the tube reaches the left side of the heart, X-ray is used to guide a thinner catheter into the left atrial appendage. The tiny device is then passed through the tube and into the appendage. When the doctor has made sure it is in the right place, she takes the catheter out, and the procedure is completed.

This minimally invasive procedure usually requires a hospital stay of at least one day. Normal activities may be resumed within a few days. About 45 days following implantation, a test will be done to determine whether the device has closed the left atrial appendage. Check-ups have to be performed every year to make sure the device is in place.

Dr. Willems is top in his field. I misunderstood about follow-up visits so had not seen him in 2 years. He was up to date on my history and recent labs, doctor visits, etc. The only glitch in my appointment was that the EKG machine was on the fritz. It wouldn't print out. That was difficult for the tech because she was very good at her job and it was upsetting for her that I had to wait. She went above and beyond to find a timely solution and I was able to make my next appointment on time. It's a pleasure to be under the care of such a talented and compassionate cardiologist who has surrounded himself with an incredible staff. Please pass along my compliments.

It was my first visit w/ Dr. Willems& before this, I didn't what to expect. I was still wishing to see my previous provider. When I saw him I felt at ease, there was a good vibe in the room that made me feel good. He came in the room with a smile in his face, introduced himself, we went over my lab results, explained the abnormal result, adviced me to drink more water especially when I go for the lab work that he scheduled, he explained my concern about the tiny murmur in my heart after it has been repaired. I'm soo thankful that I got great cardiac doctors.

Dr. Willems is one of the best, likely the best physician I have ever had. This is why I followed him from the Polyclinic to PacMed when he moved his practice. He is jovial and serious at the same time, which I seldom see in other physicians. My wife went with me to my last appointment. She was so impressed that she is going to switch to him as her cardiologist as well. We both have minor heart issues, but should they ever become serious, I will trust in his decisions with complete confidence, which is the I highest praise I can give any doctor.

This was my first visit to Dr Willems after my long time cardiologist retired. I was very impressed by several things:He was familiar with the major issues I've had with my cardiovascular history over the years, seemingly in detail; he was open and insightful in discussing concerns I've had and he had well supported suggestions in response to them; finally, he seems to me to be genuinely caring and warm and respectful of my preferences. I'm very pleased to have been referred to him.

I have seen multiple Drs this year to try to figure out what is going on. I have seen primary care drs, naturopaths, hormone specialists, ER doctors, another cardiologist, gastroenterologist etc. James Willems is by far the best DR I have had the pleasure of working with. He is kind, by far the best bedside manner, he is not dismissive, he really listens and I can tell knows his stuff. I can't say enough about him. He is just great. I wish they were all like him!

Dr. Wilhems is great and I am very happy with the care I receive from him. However, this time I was made to wait for 30 minutes in the examination room before he could come to me. He had a good excuse - another complex patient / case that needed attention before he could get to me. I would have appreciated a nurse, or someone from the practice, walking in and giving me a heads up though instead of leaving me hanging in the room. Other than that, good experience.

Doctor was excellent. Provided a complete second opinion and offered advice that fit my particular needs. I learned new things about my condition. I'd easily say he is one of the best doctors I've ever seen. This was my first visit and I'm delighted to be in his care. Please relay that I tested negative for Covid. The receptionist was concerned because my husband was within the two week period of having Covid, but he'd tested negative twice in a timely manner.

Dr. Willems presented as extremely knowledgeable and I left feeling very positive about the treatment I am receiving. Dr. Willems took the time to fully explain the SVT I am experiencing and set a course to determine which cells may be involved. He went over my EKG and set me up with a Barty which will monitor my heart rate 24/7 for the next two weeks. A nuclear stress test will be set up and I look forward to the results of the upcoming tests.

First, there was a flood that morning, which necessitated weird access to the building. All employees were extremely helpful and accommodating in getting all patients to their proper destinations. Second, Dr. Willems was amazing, which I don't say often. He was totally clear in both his questions and his explanations, as well as the way forward. I would recommend him to anyone looking for a cardiologist. I feel very fortunate having seen him.

I filled out the e-check in noting change in insurance but the receptionist didn't seem to have the information. Dr. Willems had thoroughly read my records and was very up to date on changes that had occurred in my health. He was prepared to help me with decision making from his view as my cardiologist. I always look forward to talking with him as he includes me in the decision making for the best interest of my health.
