Hyperhidrosis (Excessive Sweating)

Hyperhidrosis is a medical condition where the sweat glands become overactive, leading to excessive sweating. It can occur in just one area, such as the armpits, feet, or hands; or it can be general sweating over the entire body. Hyperhidrosis can happen to people of all ages and genders. It is estimated that the condition affects as many as 3% of all people, but many are never diagnosed because they are too embarrassed to talk about their symptoms with their doctor.

When excessive sweating is the main complaint and is not related to any other condition, it is called primary hyperhidrosis. There is no known cause, but it does tend to run in families. Secondary hyperhidrosis is sweating that is caused by another medical condition, such as anxiety, cancer, hyperthyroidism, or menopause. Sometimes treating the underlying condition can improve this type of sweating.

There are tests doctors can use to diagnose hyperhidrosis. One involves simply soaking up the perspiration with paper and then weighing the paper to measure how much extra sweat is being produced. Another test, called the starch-iodine test, involves covering the body with powder. This powder turns blue in any areas where sweat is produced.

Treatment depends on the severity of the hyperhidrosis and the areas affected. Some options are:

  • Prescription antiperspirants, which contain aluminum chloride and work by plugging the sweat ducts. This signals the sweat glands to decrease sweat production. They work for moderate cases of hyperhidrosis and are relatively affordable.
  • Lontophoresis, a procedure that uses small amounts of electricity and a tray of tap water to decrease production of sweat glands in the hands or feet.
  • Botox, which can be injected into the nerves that control sweat glands in the underarms.
  • Sympathectomy, a surgery for severe cases that can be performed to cut the nerves that stimulate sweat production. Sympathectomy is most effective for hyperhidrosis of the hands. It may cause new symptoms of sweating elsewhere on the body.
  • Surgery to remove sweat glands, most often in the underarms.

Hyperhidrosis is a serious medical condition that causes both physical and emotional distress. Treatments are available, and support groups exist to help patients cope.

Lung Issues

Every cell in the body requires oxygen to function and produces carbon dioxide as a waste product. In order to keep the body's tissues constantly supplied with fresh oxygen and to keep waste from building up, the lungs pump air in and out, even while sleeping or unaware. Unfortunately, diseases and disorders affecting the lungs are some of the most common medical problems afflicting people around the world. Some of the most common lung problems include asthma, COPD, pneumonia, and lung cancer.

Asthma is a chronic disease where swollen, inflamed airways make it hard to breathe. It is often brought on by specific triggers. While asthma is a common condition, affecting millions of people, it can also be serious and even fatal in severe cases. Treatment involves avoiding triggers and using inhaled corticosteroid medications.

COPD, or Chronic Obstructive Pulmonary Disease, is the name given to two illnesses that both cause difficulty breathing: emphysema and chronic bronchitis. In emphysema, the air sacs of the lungs become damaged, while with bronchitis they become clogged with mucus. COPD is the fourth leading cause of death in the United States. It is almost always caused by breathing irritants into the lungs, such as smoking or heavy pollution.

Pneumonia is a lung infection that can be caused by bacteria, a virus, or fungi. Most healthy people recover from pneumonia in a week or two, but for some people pneumonia can be fatal. There are treatments available for pneumonia, but the best treatment is to prevent getting sick in the first place. People who are at risk should make sure they get a flu shot every year. There is also a vaccine available for the type of bacteria that causes pneumonia.

Lung cancer occurs when abnormal cells inside the lung grow into a tumor, destroying healthy tissue. Because it is so difficult to detect lung cancer in early stages when it has few symptoms, it is the deadliest form of cancer, responsible for the most cancer deaths both in the U.S. and worldwide. Most lung cancer is caused by exposure to tobacco smoke, but between 10-15% of cases are not linked to smoking.

Some problems with the lungs are minor and are not cause for concern. However if symptoms have lasted for a month or more, even if they are minor, it is worth checking with a healthcare professional. A cough, shortness of breath, or excess mucus production that lingers for weeks are all worth getting looked at.

Tracheostomy

Tracheostomy or tracheotomy is a surgical procedure to open an airway in a blocked trachea, or windpipe. A small hole is created in the neck, and a tube is often inserted into the hole to provide support and drainage. The patient can breathe through the hole that is created.

Tracheostomy is performed for a variety of reasons when a bypass to the normal airway is needed. A patient may have an object lodged in their throat or have an injury that damaged their windpipe. They may have cancer or a swelling in their throat that stops air from moving freely. Sometimes tracheostomy is performed to make breathing easier when patients are paralyzed or have trouble coming off a ventilator.

A tracheostomy can be temporary or permanent. A temporary tracheostomy tends to heal over easily when the tube is removed, leaving a small scar. A permanent tracheostomy can make speech difficult at first. It takes time to learn how to use the throat and air in a new way.

Tracheostomy is a fairly simple procedure, but it can make all the difference in a life or death situation.

Dr. Bograd is an incredibly thoughtful and thorough surgeon. He trained at a top national cancer center and is very familiar with challenges associated with complex cancer surgeries. He spent a long time with me carefully reviewing my case, the imaging studies, surgical approach, and answering my questions. He treated both me and his staff with respect, which was a big change from my experience with a different surgeon at a different institution. He was spent time with my husband and me both before and after my surgery to answer questions and discuss my surgery. He went out of his way to make sure the pathology was done correctly. All of my other providers said he did a fantastic job. I feel very fortunate to have him in my corner.

Dr. Bograd is very concern about my health. At first I am not convince to undergo the surgery (Heller Laparoscopic Myotomy with Funduplication). I was terrified. But with his kind words and the way he explains with professionalism, I consented. I am glad I did it.I think you can trust your life to him. He is respectful and listens to our statements and inquiries. He talks to me and my spouse. He is very welcoming to know what's my status, gaining weight or losing weight, experiencing acid reflux or not. He encourages me to what type of food my palate allows, with the reservation to take it easy, small portions and swallow my food with liquid. Love our conversation, makes our day!.

Great care. When I explained I was also involved in clinical care, he took me at face value, and talked to me as a colleague. Giving me real information that I could understand (as opposed to someone he needed to speak non-clinically to), to someone who didn't have my body of experience. It was greatly appreciated. In addition, I was accompanied by my daughter (who is not a clinician), and he took the time (in different language), to explain it all to her.Very unexpectedly diagnosed with lung cancer (young non-smoker, 50s, no real risk factors), so totally this totally came from left field It was incredibly reassuring to have Dr. Bogard assigned to my care

I was actually surprised at how open to my questions about my condition this surgeon was. I had not been able to talk to anybody about my condition before seeing him, and I really had limited knowledge, which was reflected in my basic questions. One doesn't want to consult Dr. Google all the time, so I appreciated being about to start very close to square one to get pertinent, individualized information about this newly diagnosed condition. I'm 77 y.o. And have a lot of professional medical background. I tell you this so when I say that Dr. Bograd is THE best ever you'll take this seriously. He works with me and other doctors who have dealt with issues in the last year. He gave a referral when asked. I kinda/sorta teased him that I wish he would give up thoracic medicine and become my PCP. He's incredibly thorough. I consider myself very lucky.
